Collect and send the following documents:
1) Valid US Passport
You must send your valid, signed US Passport. Make sure your passport is still valid for at least 6 months, and has at least two blank visa pages. If not, you need to renew your passport first.
Foreign passport holders: In addition you must send a copy of your valid Green Card or U.S. Long Term Visa, Holders of U.S. short term tourist/business visas cannot apply in the USA.
2) China Online Visa Application (COVA) Form
Click here to complete the visa application form online. Make sure all fields are fully completed. The completed form should be printed out single sided, and signed by the applicant.
• Choose the correct Chinese consulate per your jurisdiction
• Sign the confirmation page and application where indicated
3) Passport Photograph
You must provide one recent passport photograph. photo must be in color with a plain light or white background and full front view of face.
4) Company Letter
A letter from the relevant company (i.e. airline/transport company) on the company letterhead addressed to the china consulate, It should contain the applicants name, position, and request for a visa.
5) Work ID
A copy of your work Id/Crew Badge.
6) Where You Stay Form
Print, Complete and sign the Where you stay form.
7) Proof of Residence
A copy of your state ID, utility bill or bank statement is required.

• Credit card information should be entered on the Service Order Form (See Below).
7) Service Order Form
• Complete the Service Order Form. Print a copy and send it along with rest of your documents.
8) Shipping
