Standard Services - Add Visa Pages to Passport

1)Complete the Application Form (DS-4085)

2)Valid Passport

Your passport must still be valid for at leats one year.

3)Letter of Authorization

Print and fill out the Passport Processing Authorization Letter.

4)Proof of Departure

You must provide a copy of ONE of the following:

• your foreign travel plane ticket

• your printed travel itinerary showing foreign travel

• If this is a business trip, you may present a business letter from your employer.

5)Fees and Customer Information Form

Government Fees

•Be prepared to make a personal check or Money Order for $142 payable to U.S. Department of State.

Our Fees

• Our Service Fee is $75.

• For return shipping Fee, choose one of the following.

      * A prepaid, filled in airbill from Federal Express.
      * $25 for priority overnight.
      * $20 for standard overnight.

• If you're paying by check or money order, please make the check payable to "Deluxe Passport Express".

• If you're paying by credit card, please make sure you entered your credit card information on the customer information form.(See Below)

Customer Information Form

• Complete and submit the customer information form. Once you submitted the form, print a copy and send it along with rest of the documents to Deluxe Passport Express. If you're paying by credit card make sure to sign the printed customer form.

6)Shipping

Ship the forms via FedEx using Priority overnight.

Prior to shipping, verify that you have collected the following documents:

1. Application Form DS - 4085

2. Valid Passport

3. Passport Processing Authorization Letter

4. Travel Documentation

5. Two Checks ($142 to U.S. Department of State and $75 to Deluxe Passport Express.)

6. Return prepaid airbill or shipping fee.

Place all documents in a Priority Letter.
