1)Complete and Print the Application Form (DS-4085)

2) Passport
Your passport must still be valid for at least one year.
3)Letter of Authorization
Print and fill out the Passport Processing Authorization Letter.
4)Proof of Departure
You must provide a copy of ONE of the following:
• your foreign travel plane ticket or itinerary
• If you are driving, provide your hotel reservation
• If this is a business trip, you may present a business letter from your employer.
5)Fees
•Make a personal check or Money Order for $142 payable to U.S. Department of State.
•Our Service Fee is $100.
• For return shipping Fee, choose one of the following:
* A prepaid, filled in airbill from Federal Express
* $25 for overnight shipping
• If you're paying by check or money order, please make the check payable to "Deluxe Passport Express".
• If you're paying by credit card, please make sure to enter your credit card information on the Services Order Form. (See Below)
6)Services Order Form
• Complete the Services Order Form. Print a copy and send it along with rest of your documents.
7)Shipping
Ship the following via FedEx using Priority overnight:
1. Application Form DS-4085, signed
2. Valid Passport
3. Passport Processing Authorization Letter
4. Travel Documentation
5. Two Checks ($142 to U.S. Department of State and $100 to Deluxe Passport Express)
6. Return prepaid airbill or $25 shipping fee
Call us at 718-387-5441 that you're processing a Quick Passport. Overnight it to Deluxe Passport Express, 57 Lee Ave, Brooklyn NY 11211. Our office hours are Monday through Thursday 9:00am to 6:00 pm and Friday from 9:00am to 1:00 pm EST.
• Please have your documents when calling so we can review them with you prior to shipping.
We strongly recommend that you send your documents via FedEx overnight.
