1)Complete the Application Form (DSP-82)
You have a choice of downloading the application, going to a Passport Acceptence Facility or have us fax it to you.
Do not sign this document until you are instructed to during step 8.
2)Expired Passport
• Your expired passport must have been issued less than 15 years ago.
• If your passport was issued greater than 15 years ago, you must apply for a new passport, click here for instructions.
• Your expired passport must have been issued when you were 16 or older.
• If your passport was issued when you were younger than 16, you ust apply for a new passport, click here for instructions.
• If the name you are currently using differs from the name on the expired passport, click here for instructions.
3)Passport Photographs
You must provide two identical passport photos that meet Passport Photograph Regulations.
4)Letter of Authorization
Print and fill out the Passport Processing Authorization Letter.
5)Proof of Departure
You must provide a copy of ONE of the following :
• your foreign travel plane ticket
• your printed travel itinerary showing foreign travel
6)Fees and Order Form
Government Fees
• Make a personal check or Money Order for $115 payable to U.S. Passport Services.
• Be sure to include your date of birth on this check.
Our Fees
• Our Service Fee is $100.
• Our Return Mail Fee is $15 for Priority Overnight OR you may include a pre-paid airbill to the return address.
• Our Total Fee is $115 ($100 if you INCLUDED a pre-paid airbill). You may pay by check or money order.
• If you are paying by check, please make the check payable to "Deluxe Passport Express".
7)Shipping
Ship the forms via FedEx using Priority overnight.
Prior to shipping, verify that you have collected the following documents :
1. Application Form DSP -82 (both pages, if downloaded)
2. Expired Passport
3. Two Passport Photographs
4. Passport Processing Authorization Letter
5. Travel Documentation
6. Two Checks ($115 to US Passport Services and $100 to Deluxe Passport Express)
7. Return Airbill or $15 payment to Deluxe Passport Express
Call us at 718-387-5441 and indicate you are processing an Emergency Passport from the Internet. Overnight the Priority Letter to Deluxe Passport Express, 136 Ross Street, Brooklyn NY 11211. Our office hours are Monday through Thursday 9:00am to 6:00 pm and Friday from 9:00am to 1:00 pm EST.
• Please have your documents when calling so we can review them with you prior to shipping.
We strongly suggest you use a Priority Letter to send your package should it become lost.
