Rush Passport Renewal

1)Complete and Print Application Form (DS-82)

• Click here to complete the application form online.

2)Passport

• Your passport must be issued less than 15 years ago. If it wasn't, you must apply for a new passport, click here for instructions.

• Your passport must be issued when you were 16 or older. If you were younger, you must apply for a new passport, click here for instructions.

• If your name has changed since your passport was issued:
Enclose a certified copy of legal document specifying your name change (e.g. marriage certificate, divorce, adoption decree, or court order)


3)Passport Photographs

You must provide two identical passport photos that meet Passport Photograph Regulations.


4)Letter of Authorization

Print and fill out the Passport Processing Authorization Letter.


5)Proof of Departure

You must provide a copy of ONE of the following:

•your foreign travel plane ticket or itinerary

•If you are driving, provide your hotel reservation

•If this is a business trip, you may present a business letter from your employer


6)Fees

Government Fees

•Make a personal check or Money Order for $170 payable to U.S. Department of State.


Our Fees

• Our Service Fee is $100

• For return shipping Fee, choose one of the following:

      * A prepaid, filled in airbill from Federal Express
      * $25 for  overnight shipping

• If you're paying by check or money order, please make the check payable to "Deluxe Passport Express".

• If you're paying by credit card, please make sure you entered your credit card information on the Services Order Form. (See Below)

7)Services Order Form

• Complete the Service Order Form. Print a copy and send it along with rest of your documents.

8)Shipping

Ship the following via FedEx using Priority overnight:

1. Signed Application Form DS-82

2. Passport

3. Two Passport Photographs

4. Letter of Authorization

5. Travel Documentation

6. Two Checks ($170 to U.S. Department of State and $100 to Deluxe Passport Express)

7. Return prepaid airbill or $25 shipping fee
