1)Complete the Application Form (DSP-82)

2)Expired Passport
• Your expired passport must have been issued less than 15 years ago.
• If your passport was issued greater than 15 years ago, you must apply for a new passport, click here for instructions.
• Your expired passport must have been issued when you were 16 or older.
• If your passport was issued when you were younger than 16, you must apply for a new passport, click here for instructions.
• If your name has changed since your passport was issued:
Enclose a certified copy of legal document specifying your name change (e.g. marriage certificate, divorce, adoption decree, or court order)
3)Passport Photographs
You must provide two identical passport photos that meet Passport Photograph Regulations.
4)Letter of Authorization
Print and fill out the Passport Processing Authorization Letter.
5)Proof of Departure
You must provide a copy of ONE of the following :
• your foreign travel plane ticket
• your printed travel itinerary showing foreign travel
6)Fees and Customer Information Form
• Be prepared to make a personal check or Money Order for $135 payable to U.S. Department of State.
• Be sure to include your date of birth on this check.
• Our Service Fee is $125.
• For return shipping Fee, choose one of the following.
* A prepaid, filled in airbill from Federal Expres.
* $25 for priority overnight.
* $20 for standard overnight.
• If you're paying by check or money order, please make the check payable to "Deluxe Passport Express".
• If you're paying by credit card, please make sure you entered your credit card information on the customer information form.(See Below)
• Complete and submit the customer information form. Once you submitted the form, print a copy and send it along with rest of the documents to Deluxe Passport Express. If you're paying by credit card make sure to sign the printed customer form.
7)Shipping
Ship the forms via FedEx using Priority overnight.
Prior to shipping, verify that you have collected the following documents :
1. Application Form DSP -82
2. Expired Passport
3. Two Passport Photographs
4. Passport Processing Authorization Letter
5. Travel Documentation
6. Two Checks ($135 to U.S. Department of State and $125 to Deluxe Passport Express)
7. Return prepaid airbill or shipping fee.
Call us at 718-387-5441 to indicate that you're processing an Urgent Passport from the Internet. Overnight the Priority Letter to Deluxe Passport Express, 57 Lee Ave, Brooklyn NY 11211. Our office hours are Monday through Thursday 9:00am to 6:00 pm and Friday from 9:00am to 1:00 pm EST.
• Please have your documents when calling so we can review them with you prior to shipping.
We strongly recommend that you send your documents via FedEx overnight.
