Instructions to obtain a Rush Passport Renewal
1) Complete and Print Application Form DS-82
• Click here to complete the application form online.
2) U.S. Passport
• Your passport should be issued within the past 15 years.
• You must've been over the age of 16 at the time your passport was issued.
• If your name has changed since your passport was issued, present a certified copy of legal document specifying your name change (e.g. marriage certificate, divorce, adoption decree, or court order)
3) Passport Photo
You must provide one 2"x2" passport photo. See U.S. Passport
Photograph Regulations.
4) Letter of Authorization
Print and fill out 2 Passport Authorization Letters.
5) Proof of Travel
You must provide a copy of ONE
of the following :
• Printout of your foreign
travel booking
• If this is
a business trip, present a business letter from
your employer
• If you are driving, provide your hotel reservation
6) Fees
• Make a personal check or Money Order for $170 payable to U.S. Department of State. (Starter's checks not accepted)
• Our Service Fee is $125
• For return
delivery, add $25 for overnight shipping
or provide your FedEx account # on the order form (below).
• Credit card information should be entered on the order form (below).
• When paying by check/money order, make it payable to "Deluxe Passport Express".
7) Services Order Form
• Complete the Service Order Form. Print a copy and send it along with rest of your documents.
8) Shipping
Visit one of our walk-in offices, or ship the
following via FedEx/UPS:
1. Signed Application Form DS-82
2. Passport
3. Passport Photo
4. 2 Letters of Authorization
5. Travel Document
6. Government fee payment
7. Service fee and shipping payment
All signed documents must have original signature.
Call 718-387-5441 to inform us that you're processing a Rush Passport. Overnight it to Deluxe Passport Express, 57 Lee Ave, Brooklyn NY 11211. Our office hours are Monday through Thursday 9:00am to 6:00pm, and Friday from 9:00am to 1:00pm EST.
