US Passport Name Change Expedited Service
1)Complete and Print the Application Form (DS-5504)

2)Passport
Make sure your passport is still valid, and was issued less than one year ago. If not, click here for Renewal instructions.
3)Passport Photograph
You must provide one 2" x 2" passport photo that meets Passport Photograph Regulations.
4)Letter of Authorization
Print and fill out 2 Passport Authorization Letters.
5)Proof of Travel
You must provide a copy of ONE
of the following :
• Printout of your foreign
travel booking
• If this is
a business trip, present a business letter from
your employer
• If you are driving, provide your hotel reservation
6)Proof of Name Change
To prove your name change, you are required to provide official documentation certifying the change. Acceptable documents are a Marriage Certificate or a Court Order.
• The name change document MUST bear the official seal of the issuing authority.
• Uncertified copies or notarized documents are UNACCEPTABLE.
• The document has to be issued after your passport's issue date.
7)Fees
• Make a personal check or Money Order for $60 payable to U.S. Department of State. (Starter's checks not acceptable)
• Our Service Fee is $75.
• For return
delivery, add $25 for overnight shipping
or provide your FedEx account # on the order form (see below).
• Credit card information should be entered on the order form(see below).
• When paying by check/money order, make it payable to "Deluxe Passport Express".
• Complete the Services Order Form. Print a copy and send it along with rest of your documents.
9)Shipping
Visit one of our walk-in offices, or ship the
following via FedEx/UPS:
1. Signed Application Form DS-5504
2. Valid Passport
3. 2 Letters of Authorization
4. Travel Documentation
5. Proof of Name Change
6. Passport Photo
7. Government Fee Payment
8. Service Fee and Shipping Payment
All signed documents must have the original signature.
Call us at 718-387-5441 that you're processing an Expedited Passport. Overnight it to Deluxe Passport Express, 57 Lee Ave, Brooklyn NY 11211. Our office hours are Monday through Thursday 9:00am to 6:00 pm and Friday from 9:00am to 1:00 pm EST.
