1)Complete the Application Form (DS-5504)

2)Valid Passport
Make sure your passport was issued less than one year ago.

3)Passport Photographs
You must provide two identical passport photos that meet Passport Photograph Regulations.
4)Letter of Authorization
Print and fill out the Passport Processing Authorization Letter.
5)Proof of Departure
You must provide a copy of ONE of the following :
• your foreign travel plane ticket
• your printed travel itinerary showing foreign travel
• If this is a business trip, you may present a business letter from your employer.
6)Proof of Name Change
To prove your name change, you are required to provide official documentation certifying the change. The two official documents accepted are a Marriage Certificate or a Court Order.
• The name change document MUST bear the official seal of the issuing authority.
• Uncertified copies or notarized documents are UNACCEPTABLE.
• Has to be issued after your passport issue date.
7)Fees and Customer Information Form
• Be prepared to make a personal check or Money Order for $60 payable to U.S. Department of State.
• Be sure to include your date of birth on this check.
• Our Service Fee is $100.
•For return shipping Fee, choose one of the following
* A prepaid, filled in airbill from Federal Expres.
* $25 for priority overnight.
* $20 for standard overnight.
• If you're paying by check or money order, please make the check payable to "Deluxe Passport Express".
• If you're paying by credit card, please make sure you entered your credit card information on the customer information form.(See Below)
• Complete and submit the customer information form. Once you submitted the form, print a copy and send it along with rest of the documents to Deluxe Passport Express. If you're paying by credit card make sure to sign the printed customer form.
8)Shipping
Ship the forms via FedEx using Priority overnight.
Prior to shipping, verify that you have collected the following documents :
1. Application Form DS-5504
2. Valid Passport
3. Passport Processing Authorization Letter
4. Travel Documentation
5. Proof of Name Change
6. 2 Passport Photos
7. Two Checks ($60 to U.S. Department of State and $100 to Deluxe Passport Express)
8. Return Airbill or payment to Deluxe Passport Express.
