Do not sign the DS-11 and DS-64 document until you are instructed to during step 8 .

2)Proof of U.S. Citizenship

You must provide an official copy of ONE of the following documents :
If you were born in the United States :

• Original Birth Certificate issued by city, county or state

• Previous U.S. Passport

If you were born outside the United States, click here to see your options.

3)Proof of Identity

You must provide an official copy of ONE of the following documents :

• Copy of current, valid Driver's License with Photo (must be issued greater than 6 months ago)

• If the above mentioned Driver's License isn't available, then you need to supply an alternate Proof of Identity.

4)Passport Photographs

You must provide two identical passport photos that meet Passport Photograph Regulations.

5)Letter of Authorization

Print and fill out the Passport Processing Authorization Letter.

6)Proof of Departure

You must provide a copy of ONE of the following :

• your foreign travel plane ticket

• your printed travel itinerary showing foreign travel

• If this is a business trip, you may present a business letter from your employer.

7) Fees and Customer Information Form

Government Fees

• Be prepared to make a personal check or Money Order for $127 payable to U.S. Department of State.

• Be sure to include your date of birth on this check.

• This check will go into a sealed, government envelope during step 8.

• Be prepared to pay the Acceptance Agent $30 by Cash, Check or Money Order.

Our Fees

• Our Service Fee is $150.

• For return shipping Fee, choose one of the following

       * A prepaid, filled in airbill from Federal Expres.
       * $20 for priority overnight.
       * $15 for standard overnight.

• If you're paying by check or money order, please make the check payable to "Deluxe Passport Express".

• If you're paying by credit card, please make sure you entered your credit card information on the customer information form.(See Below)

Customer Information Form

• Complete and submit the customer information form. Once you submitted the form, print a copy and send it along with rest of the documents to Deluxe Passport Express. If you're paying by credit card make sure to sign the printed customer form.

8)Authorization

Prior to authorization, verify that you have collected the following documents:

1. Application Form DS-11

2. Application Form DS-64

3. Proof of Citizenship

4. Proof of Identity

5. Two Passport Photographs

6. Passport Processing Authorization Letter

7. Travel Documentation

8. Government Fees

Make copies of all documents mentioned above.

You must appear in person at a court house or post office and present the original documents to an acceptance agent which is authorized to accept passport applications. The acceptance agent is needed to witness the signing of your application for the passport.

Notify the agent you are using an expediting service to "hand carry" your application. Most facilities are open M-F, 9am to 330pm. The acceptance agent will take about ten minutes to go through the following steps :

1. To verify your identity, the agent will request your proof of identity.

2. The agent will ask you to sign the DS-11 and DS-64.

3. To verify that you need an expedited passport, you will need to show them your travel documentation.

4. The agent will then seal the DS-11 and DS-64, photos, proof of citizenship, travel documents, authorization letter and the $127 check in an envelope marked "To be Opened by U.S. Passport Office Only".

5. Important: Make sure the agent returns the sealed envelope to you. If the agent does not feel at ease about releasing the envelope, suggest that he/she refer to chapter 10 of their Passport Agent's Manual, "Hand-carrying of Executed Applications".

Do not open this sealed envelope or the application will be invalid.

9)Shipping

Ship the forms via FedEx using Priority overnight.

1. Place the following documents in a Priority Letter:

• Sealed Government Envelope

• Payment to Deluxe Passport Express for $150

• Return Airbill or $20 payment to Deluxe Passport Express

• Copies of all Documents to the list add

• Customer information form:
