Extend Limited Passport Validity

1)Complete and Print Application Form DS-5504

2)Passport

• You must provide your limited US Passport. The passport must be valid and issued less than one year ago.

• You must submit a certified copy of all documents, which were missing at the time when you applied for your limited passport (e.g. birth certificate, proof of US citizenship etc.)

3)Passport Photo

• You must take a passport photo that meets Passport Photograph Regulations.

4) Letter of Authorization

Print, complete, and sign the Passport Authorization Letter.

5) Proof of Travel

You must provide a copy of ONE of the following:

• Printout of your foreign travel booking

• If you are driving, provide your hotel reservation

• If this is a business trip, you may present a business letter from your employer.

6)Fees

Government Fees

• Make a personal check or Money Order for $60 payable to U.S. Department of State. (starter's checks not accepted)


Our Fees

• Our Service Fee is $75.

• For return delivery, add $25 for overnight shipping or provide your FedEx account # on the order form (see below).

•Credit card information should be entered on the order form.(see below)

• When paying by check/money order, make it payable to "Deluxe Passport Express".

7)Services Order Form

• Complete the Services Order Form. Print a copy and send it along with the rest of your documents.

8)Shipping

Visit one of our walk-in offices, or ship the following via FedEx/UPS:

1. Signed Application Form DS-5504

2. Passport photo

3. Valid Passport with supporting documents

4. Letter of Authorization

5. Travel Document

6. Government fee payment

7. Service fee and shipping payment
