1)Complete the Application Form (DS-5504)

2)Valid Passport
• You must provide your limited US Passport. The passport must be valid and issued less than one year ago.
• You must submit a certified copy of all documents, which were missing at the time when you applied for your limited passport (e.g. birth certificate, proof of US citizenship etc.)
3)Passport Photographs
• You must provide two identical passport photos that meet Passport Photograph Regulations.
4) Letter of Authorization
Print and fill out the Passport Processing Authorization Letter.
5) Proof of Departure
You must provide a copy of ONE of the following:
• Your foreign travel plane ticket.
• Your printed travel itinerary showing foreign travel.
• If this is a business trip, you may present a business letter from your employer.
6)Fees and Customer Information Form
• Be prepared to make a personal check or Money Order for $60 payable to U.S. Department of State.
• Be sure to include your date of birth on this check.
• Our Service Fee is $150.
• For return shipping Fee, choose one of the following.
* A prepaid, filled in airbill from Federal Express.
* $20 for priority overnight.
* $15 for standard overnight.
• If you're paying by check or money order, please make the check payable to "Deluxe Passport Express".
• If you're paying by credit card, please make sure you entered your credit card information on the customer information form.(See Below)
• Complete and submit the customer information form. Once you submitted the form, print a copy and send it along with rest of the documents to Deluxe Passport Express. If you're paying by credit card make sure to sign the printed customer form.
7) Shipping
Ship the forms via FedEx using Priority overnight.
Prior to shipping, verify that you have collected the following documents:
1. Application Form DS-5504
2.Two passport photos
3. Valid Passport and all documents
4. Passport Processing Authorization Letter
5. Travel Documentation
6. Two Checks ($60 to U.S. Department of State and $150 to Deluxe Passport Express)
7. Return Airbill or $20 payment to Deluxe Passport Express
Call us at 718-387-5441 to indicate that you're processing an Emergency Passport from the Internet. Overnight the Priority Letter to Deluxe Passport Express, 57 Lee Ave, Brooklyn NY 11211. Our office hours are Monday through Thursday 9:00am to 6:00 pm and Friday from 9:00am to 1:00 pm EST.
Please have your documents when calling so we can review them with you prior to shipping.
We strongly recommend that you send your documents via FedEx overnight.
