EXPEDITE YOUR APPLICATION FOR A CHILD OR INFANT PASSPORT

Child/Infant Passport Standard Services.

Standard processing of a child or infant passport usually takes 5 - 7 days. If you need faster service, consider our urgent passport processing or rush passport processing service to obtain a child or infant passport in a little as 24 hours.

1)Complete the Application Form (DSP-11)

• Click here to download the application form DS-11

• If your child previously had a U.S. passport, You must submit the passport.

• If the passport is lost or stolen, You need to fill out lost application
form DS-64, Click here to download application.

Do not sign this document until you are instructed to during step 8.

2)Proof of U.S. Citizenship

You must submit the child's Original Birth Certificate issued by city, county or state:

If your child was born outside the United States, click here to see your options.

3)Parent's Proof of Identity

You must provide an official copy of ONE of the following documents :

• Both parent's current, valid Driver's License with Photo (must be issued greater than 6 months ago) or valid passport.

• If the above mentioned Identification isn't available, then you need to supply an alternate Proof of Identity.

4)Passport Photographs

You must provide two identical passport photos that meet Passport Photograph Regulations.

5) Letter of Authorization

Print and fill out the Child Passport Processing Authorization Letter.

6) Proof of Departure

You must provide a copy of ONE of the following :

• your foreign travel plane ticket

• your printed travel itinerary showing foreign travel

• If this is a business trip, you may present a business letter from your employer.

7) Fees and Customer Information Form

Government Fees

• Be prepared to make a personal check or Money Order for $120 payable to U.S. Department of State.

• Be sure to include your date of birth on this check.

• This check will go into a sealed, government envelope during step 8.

• Be prepared to pay the Acceptance Agent $25 by Cash, Check or Money Order.

Our Fees

• Our Service Fee is $100.

• For return shipping Fee, choose one of the following.

        * A prepaid, filled in airbill from Federal Expres.
        * $25 for priority overnight.
        * $20 for standard overnight.

• If you're paying by check or money order, please make the check payable to "Deluxe Passport Express".

• If you're paying by credit card, please make sure you entered your credit card information on the customer information form.(See Below)

Customer Information Form

• Complete and submit the customer information form. Once you submitted the form, print a copy and send it along with rest of the documents to Deluxe Passport Express. If you're paying by credit card make sure to sign the printed customer form.

8)Authorization

Prior to authorization, verify that you have collected the following documents

1. Application Forms

2. Proof of Citizenship

3. Proof of Identity

4. Two Passport Photographs

5. Passport Processing Authorization Letter

6. Travel Documentation

7. Government Fees

Make copies of all documents mentioned above.

Minors under 16 must appear in person with both parents at a post office or court house, And present the original documents to a passport acceptance agent. If one of the parents is not available to appear in person, The may submit a Statement of Consent Form. This form should be completed, signed, and notatized by the non-applying parent, and should be submitted with the rest of the documents. For children over 16, only one parent is required to appear with the child.

The acceptance agent is needed to witness the signing of your application.

Notify the agent you are using an expediting service to "hand carry" your application. Your child must go along to the post office. If the child is over 14 only one parent is required. Most facilities are open M-F, 9am to 330pm. The acceptance agent will take about ten minutes to go through the following steps :

1. To verify your identity, the agent will request your parent proof of identity.

2. The agent will ask you to sign all the forms.

3. To verify that your child needs an expedited passport, you will need to show them your child's travel documentation.

4. The agent will then seal the forms, photos, proof of citizenship, travel documents, authorization letter and the $120 check in an envelope marked "To be Opened by U.S. Passport Office Only".

5. Important: Make sure the agent returns the sealed envelope to you. If the agent does not feel at ease about releasing the envelope, suggest that he/she refer to page 31 of their Passport Agent's Manual, "Hand-carrying of Executed Applications".

Do not open this sealed envelope or the application will be invalid.

9)Shipping

Ship the forms via FedEx using Priority overnight.

Place the following documents in a Priority Letter:

• Sealed Government Envelope

• Payment to Deluxe Passport Express for $100

• Return prepaid airbill or shipping fee.

• Copies of all Documents to the list add

• Customer information form
